1. Accomplish the “Simplified Reinstatement Form” (attached, click the title) – one (1) health declaration question!
   
   

2. Client may choose their preferred/applicable reinstatement type – Updating or Redating

· Redating: payment of one modal premium (applicable for lapsed unit-linked plans)

· Updating: payment of all missed premiums

4. Submit the accomplished SRO form through any of the ff:

   a) via email at policy.admin@prulifeuk.com.ph pluktitaniumpolicy.services@yahoo.com
   cc: Unit Manager and Unit Officer ; or

   b) Pru Life UK Customer Centers or GA offices
   

Some important notes:

- The offer is applicable only to qualified policies. Financial underwriting still applies. Please submit the form following the usual processing cut-off to ensure timely processing of reinstatement request


- The list is extracted as of May 13, 2024. It is highly encouraged that agentscheck PRISM first before reaching out to clients to know the current policy status of the policies. Kindly disregard if policy has been reinstated. Only lapsed policies can be reinstated.


- Your agents with policies qualified for the offer will also receive their list via email.


- Customers with qualified lapsed policies will also receive communications from Head Office about the offer
